Before you practice, you must have the following completed and on record with your coach:

3. Athletic Emergency Information

4. $60 Athletic Fee (Make checks payable to: George Washington High School) must be paid prior to playing in any games/matches

(All checks must have the Colorado Driver's License number of the person signing the check and the DPS ID number of the student.)

Any student who has tranfered to George Washington, from another high school regardless of when the transfer takes place,  must fill out a CHSAA transfer Contact Form.  This form can be found below and must be signed by both the athlete and a parent/guardian and be notarized.

There is inherent risks when participating in any sport, please, read the Athletic warning that can be found below.

If you are an athlete that will also be competing for a club team while participating in a sport at GW, you must complete and have on file with the athletic director an Outside Practice Permission form which can be found below.
